What Is IASTM?
IASTM physical therapy treatment (Instrument Assisted Soft Tissue Mobilization), is a type of soft tissue mobilization which is based off of the ancient Chinese medicine technique called “Gua Sha”. In ancient Chinese medicine, Gua Sha was performed by applying oil to one’s skin and then using specifically shaped stones to massage the skin.
Gua Sha is thought to increase blood flow to the area which it is applied to and therefore promote the flow of oxygen and blood to the soft tissues. We now use wood, plastic, or metal tools and lotion or oil on the affected area when performing IASTM to promote similar effects. IASTM is used for about 3-5 minutes per body part and is a great treatment to optimize function, performance, and healing.
How does Instrument Assisted Soft Tissue Mobilization (IASTM) work?
When our bodies undergo an injury to our soft tissue (muscles, ligaments, tendons, fascia) we have a 3 stage response to recover. These stages are called the inflammation stage, repair stage, and remodeling phase. During the repair stage collagen is laid down to repair the tissue quickly and unorganized. During the remodeling phase these collagen fibers are realigned and strengthened. IASTM can help during the repair and remodeling phase to increase blood flow to the area and increase the healing factors. IASTM has also been shown to stimulate the nervous system in a specific way which can help decrease pain and tone.

When to use IASTM
IASTM may be used by your physical therapist for a number of reasons. Many therapists will choose to use it in the beginning of your session to optimize your range of motion or decrease pain so you can get the most out of your session. IASTM can also be a great way to promote blood flow to the target area to prepare for your therapeutic exercise. IASTM has been clinically proven to increase outcomes for those with:
- Tendinopathies
- Acute or chronic ligament injuries
- Acute or chronic muscle injuries
- Increased muscle tone
- Post surgery
- Scar treatment
