Terminal knee extension is the final phase of straightening that brings the knee from almost straight to fully locked out. This small range of motion plays a major role in knee health and function. Everyday movements like walking, standing, and climbing stairs (plus many core athletic movements) are most efficient when the knee can fully straighten. When the knee lacks even a few degrees of extension, it can:
- Change how weight is distributed through the joint
- Increase stress on certain areas of cartilage
- Limit stability by reducing the knee’s natural locking mechanism
- Lead to compensations at the hip, back, or opposite leg
Over time, even small deficits in knee extension can have significant consequences. These deficits are associated with altered gait patterns, increased joint stress, and persistent quadriceps weakness, which in turn limit functional performance. Because the quadriceps work most effectively when the knee can fully straighten, loss of extension often leads to ongoing weakness. Chronic extension loss has also been linked to a higher risk of developing or accelerating knee osteoarthritis.
After injury or surgery, several factors can limit knee extension:
- Swelling inside the joint
- Pain and muscle guarding
- Scar tissue formation
- Hamstring tightness
- Quadriceps inhibition
In physical therapy, restoring extension is often an early priority. Treatment may include:
- Swelling management
- Manual therapy and joint mobilization
- Stretching to improve mobility
- Targeted quadriceps activation
- Neuromuscular re-education
- Gait training
